Care that puts comfort first

Hospice care is for people with a serious illness who choose comfort instead of treatment aimed at a cure. The hospice team eases pain and other symptoms and supports the family, too.

How hospice care works
What it means
Nurse visits, symptom relief, medications and medical equipment, plus support for the family.
What it doesn’t mean
That care stops. Medical care continues, and the team visits according to the care plan.
Where care happens
At home, in nursing homes and in assisted living facilities. Our team comes to your loved one wherever they live.
Who decides
The patient or their representative, with the family and doctor. The patient can change their decision at any time.

When comfort matters most

  • When treatment no longer brings relief and comfort has become the priority.
  • When a doctor recommends hospice care.
  • When your loved one would rather be cared for at home, with family nearby.

A doctor confirms who qualifies. When we call you back, we’ll walk you through the criteria.

Care for your loved one. Support for your family.

Services are available 24 hours a day, 7 days a week.

  • Nursing care

Nurse visits to manage pain, shortness of breath and other symptoms.

  • Medications and equipment

Medications for pain and symptom relief, plus medical equipment for care at home.

  • Support for family caregivers

We show you how to care for your loved one. A social worker supports the patient and family.

  • Grief support

Your family can turn to us for support and someone to talk to after a loss.

What happens after you leave a request

  1. Step 1

    You leave a request

We call you back, answer your questions and ask about your loved one.

You’ll learn whether hospice may be a good fit.

  2. Step 2

    A nurse visits

A hospice nurse visits your loved one and family to understand their needs.

You’ll know what help is needed and what comes next.

  3. Step 3

    Doctor review

The hospice physician and the patient’s own doctor, if they have one, confirm whether the patient is eligible for hospice.

Once the patient agrees, hospice care can begin.

  4. Step 4

    Your care plan

We make a plan with your loved one and family: visits, medications, equipment and who to call.

You’ll know who is coming and when.

Common questions

Does Medicare or Medi-Cal cover hospice care?

Yes, if the patient is eligible for hospice care. Medicare pays for care related to the patient’s main illness, including visits from the hospice team, medications for pain relief and symptom control, plus medical equipment and supplies. We accept Medicare Part A, Medi-Cal (Medicaid), major HMO insurance plans and private pay. When we call you back, tell us your plan, and we’ll verify coverage before care begins.

Can the patient keep seeing their own doctor?

Yes, if the patient chooses that doctor as their attending doctor for hospice care. We then work with that doctor and keep them up to date.

What if the patient’s condition changes?

The team reviews the care plan and talks through any changes with the patient, the family and the patient’s doctor. If the patient’s health improves, they may no longer need hospice care. Their own doctor then continues their care.

Do you support the patient’s family?

Yes. Supporting the family is part of hospice care. The team helps you care for your loved one, and a social worker is there for you, too. Support continues after a loss.

Do we need a doctor’s referral to contact you?

No. You can contact us yourself. We’ll answer your questions and explain how hospice care can begin.

Does choosing hospice mean giving up?

No. Hospice care shifts the focus from a cure to comfort and quality of life. The patient can stop hospice care at any time and go back to treatment for their illness.

This is general information. It doesn’t replace a doctor’s advice.
